User:Meaganl/DNA submission form

We will provide this form to the teams in order send parts to the Registry this year.  We are only accepting parts as DNA, in either dry or liquid form.  All of the fields are mandatory.  

Issues to figure out:
* instruction on how to label 96-well plates
* give instructions on how to <u>(and ensure that they will)</u> send me a digital copy of what part corresponds to which tube or give the layout of parts on a 96-well plate.
* do we want to accept parts that are not 'selected parts'?
